The following is a survey to determine how well our church is doing in the task of "Making Disciples... Teaching them to obey all things I have commanded you."(Matthew 28:19) We ask for your help in taking an honest inventory of where you stand individually. The church is made up of individuals, therefore the quality of your Christian life is a reflection of the effectiveness of the ministries in the church. I understand that this may be very difficult for some, but many of us have lived for many years as a christian without really looking at all the aspects of our life in relation to Christ. Just like standing on a scale, we must see where we are in order to determine where we need to go. If this year you only check "yes" on 5 items, then next year we hope you check "yes" on 6 items. Discipleship is a process, and we want to see you succeed in living the Christian life. This survey will remain anonymous.
